Payment Obligation. The Applicant must pay the full Fee at the time of submitting this form in order for the Provider to begin processing.

No Refund Under Any Circumstances. Once the Fee is paid, it is non-refundable. The Provider will not, under any circumstances, refund, credit, or adjust the Fee, whether or not the Applicant’s request is approved, completed, delayed, abandoned, or otherwise.
